Reform & Rejoin

Prison Reform & Second Chance Initiative

Restoring Lives. Renewing Hope. Rebuilding Futures.

Reform & Rejoin is The Tawbah Foundation’s dedicated support initiative focused on healing, rehabilitation, and reintegration for vulnerable individuals in Northern Ghana. This program exists to serve those most often forgotten—young people battling addiction, incarcerated individuals, and people living with special health conditions—offering them a pathway back to dignity, stability, and purpose.

At the heart of this initiative is a compassionate approach that combines:

  • Trauma-informed counseling and emotional support

  • Life skills and vocational training to promote independence

  • Faith-based mentorship and community outreach

  • Ongoing reintegration support for sustained change

We are particularly focused on youth affected by the growing opioid crisis in Tamale, with special attention to young women, who face not only addiction but also social stigma, isolation, and lack of access to care.

Our efforts also extend to inmates and former offenders, offering them a second chance through educational workshops, basic necessities, and post-release reintegration support. For individuals living with chronic health conditions or disabilities, we strive to create inclusive pathways to rejoin society and live meaningful lives.

Reform & Rejoin is not just about recovery—it’s about restoring dignity, building resilience, and walking with people as they reclaim their futures.
